I have a Master’s Degree of Health. Additionally, I have 10+ years of experience as a service coordinator and care coordinator to individuals with intellectual and physical disabilities, living at home or in a residential setting. I have experience assisting in developing circles of support and facilitating person‐centered plans. I have experience supervising and providing orientation, training and support to staff. I have knowledge of the New‐York State Department of Health (DOH) and the Office of People with Developmental Disabilities (OPWDD) regulations.I believe that the self‐advocacy/self‐ determination services will assist any individual to have control over the services and programs they need to become a productive part of their community.As a start‐up/support broker, I will utilize my experience to assist individuals with intellectual disabilities in defining and achieving their goals and objectives.
