Agape Healthcare Services support coordinators will work to get your loved ones services planned by using the person-centered planning process to assist individuals and their families to develop and implement individualized service plans.
Support Coordination is provided for individuals who are eligible to receive services through the NJ Division of Developmental Disabilities (DDD)
Support Coordinators will work with individuals and their families to build a working and better rapport to capture individual true personalities, unique differences, individual strengths in order to create a plan or services that will best suit their needs.
