•Services accessible through NJ Division of Developmental Disabilities (DDD) for adults 21 years and older with intellectual and developmental disabilities
• The services are supported and planned by support coordinators along with participants through person centered planning processes
• Some examples of services include supports for medical, social, employment and educational purposes
• People can become eligible for services based on the following:
• Must be 21 years of age or older
• Eligible for DDD services/ have a developmental disability
• Eligible for Medicaid
• Permanent residence in New Jersey
