Our mission at Prime Care Coordination is to partner with people with intellectual and/or developmental disabilities to receive the services and supports they need to live their lives in the most meaningful way.

The Center for Disability Services provides psychological assessments to aide in OPWDD eligibility determination. Areas that are evaluated are intellectual ability, adaptive behavior, and autism spectrum disorders.
